Helping Oppressed & Marginalized Communities to Elevate, Inc., fondly known as HOME, is proudly a 501(c)(3), grassroots organization that serves, empowers, and uplifts underserved communities. Our formation was a result of community-wide inequities in healthcare that disproportionately impact people of color, LGBTQ+ people, and people at the intersections of other identities that face health disparities rooted in social factors, such as implicit bias and discrimination. HOME, Inc. responds to those disparities and the situations in which they arise to create a healthcare system inclusive of all communities. As you will see in the clinical and national data, the United States has shown unequal treatment and care to many, different socio-economic groups and, as a result, it has led to our organization’s creation to focus on the underserved.
Services
In contract with small independent medical and mental health practices, our organization offers family medicine, counseling, telehealth, community outreach, health education, support groups, clinical social work and case management. Our services are designed to offer coordinated and comprehensive care for our priority populations based on our foundational principles and healthcare strategies.
